Request Sacramental Records

To obtain sacramental records from Holy Name Cathedral, all requests must be submitted online along with the appropriate identification.

Please click below to complete the online form. It will take approximately 14 days to complete our search and produce the documentation.

Please see the list of requirements listed below

• You need to include your name at the time of the sacrament, birth date, and date of sacrament.
• You also need to include a copy of some form of identification, such as a copy of your current state ID or driver’s license.
• Please provide a current phone number so someone from the parish office can contact you if questions arise.

There will be a $10.00 fee that can be paid via the online request form.
