Take Our 2026 Parish Survey

Our parish is on a journey to know, love, and serve Christ as a parish community. Together we must embrace Christ’s vision for missionary discipleship so our parish can thrive and be a place for others to grow together in faith.

The very first step in this process involves assessing our greatest strengths and opportunities, and that is where the Disciple Maker Index (DMI) comes in! The DMI is an important survey tool that invites you to reflect on your discipleship journey here at Holy Name Cathedral.

If you prefer a paper copy, you may pick up your survey after daily and Sunday Masses. They will be located at the Holy Name Cathedral Rectory Office. To submit the completed paper survey, please drop off at the Rectory during any of our regular hours: Mon-Sat 9:00am – 4:00pm and Sunday 9:00am – 2:00pm.

At Holy Name Cathedral, we are blessed in so many ways. Together, we will identify our strengths and opportunities in greater detail as we strive to grow in missionary discipleship.

Your Voice Matters! Parish Survey Invitation

Dear People of God, 

Three years ago, you shared your thoughts about our Faith Community, and your voices helped shape meaningful changes in how we worship, serve, and grow together. Now, we’re asking for your input once again.

We invite you to participate in our 2026 Disciple Maker Index Survey.

This survey is an opportunity to reflect on your experience at Holy Name Cathedral and help us understand how well we’re serving our Faith Community.

Your honest feedback will allow us to measure our progress since 2023 and identify new areas where we can strengthen our parish life together.

The Disciple Maker Index is a survey tool that supports Church ministry with objective data, helping leaders recognize their potential and envision future possibilities through a deep analysis of attitudes and beliefs, participation, relationships/satisfaction, and demographics.

Why Your Voice Matters

Whether you’re a young adult navigating career and life transitions or you’re in a season of creating a family or enjoying a new phase of renewed freedom and purpose, your perspective is essential. We especially want to hear from you about:

• How can we better support you in your faith journey during this formative season of life?
• How can we engage your gifts, wisdom, and availability in meaningful ways?
